1. Introduction
This document outlines the terms and conditions of the Total Vision Protection Plan ("Plan") offered by LifeArt Vision ("Company", "we", "us", or "our"). This Plan is an optional extended service protection agreement that provides specific coverage for accidental damage and manufacturing defects of eyewear products purchased directly from LifeArt Vision.
By purchasing this Plan, you ("Customer", "you", or "your") acknowledge that you have read, understood, and agreed to be bound by the terms herein.
2. Term of Coverage
The Plan is valid for a term of 365 calendar days (1 year) from the date of purchase of the covered eyewear product. Coverage terminates after one (1) approved claim or at the end of the 1-year period, whichever comes first.
3. Plan Eligibility

The Plan must be purchased at the time of the original eyewear transaction.

The Plan applies only to the specific product(s) listed on the original invoice.

Each eyewear item requires its own Plan. Plans are not transferable between products or individuals.

The Plan is only valid for products purchased directly from LifeArtVision.com
4. Pricing Structure
The Plan cost is based on the type of lenses included with the original eyewear purchase:

5. Copay Schedule (Per Approved Claim)
The Customer agrees to pay a copay based on the original price of the eyewear, excluding shipping, tax, and Plan cost:

$80 to $149 | $35 |
$150 and above | $50 |
The copay must be paid in full before the replacement eyewear is shipped. All copayments are final and non-refundable.
6. What Is Covered
This Plan provides one (1) replacement for the covered eyewear during the term of the Plan for the following conditions:

Cracked, scratched, or chipped prescription or non-prescription lenses

Frame damage caused by drops, impacts, or daily wear and tear
-
Damage due to defective hinges, screws, or frame components
-
Prescription lens degradation due to coating failure (e.g., anti-reflective, blue light filters)

Frame misalignment that impairs functionality
The replacement will be for the same frame style and lens type, subject to availability. If the original frame is no longer available, a frame of similar value and style will be offered.
7. What Is Not Covered
This Plan excludes coverage for:

Lost or stolen eyewear

Intentional or abusive damage (e.g., crushed frames, extreme heat, or deliberate misuse)

Unauthorized repairs or modifications

Claims submitted with false or incomplete information

Accessories (cases, pouches, lens cloths, cords, etc.)
-
Lenses with prescription updates differing from the original order
8. Claim Submission Process
To file a claim, the Customer must:

Email support@lifeartvision.com with the following information:

Full name and original order number

Description of the issue

Clear, high-resolution photos of the damaged eyewear (including front, back, and affected areas)

Customer Support will review the claim within 3–5 business days and respond with eligibility confirmation.

If approved, the Customer must submit payment for the applicable copay.

Once the copay is received, a replacement order will be processed and shipped to the original delivery address or an alternate address provided by the Customer.
9. Replacement Limitations

Only one (1) replacement is allowed under each Plan.

All replacement lenses must match the original prescription exactly.

Replacement orders are subject to standard production timelines.

Replacement products are covered for the remainder of the original Plan term only.
10. Cancellation and Refund Policy

The Plan may be canceled for a full refund within 14 calendar days of purchase, provided that no claim has been filed.

If the original eyewear is returned under our standard return policy, the Plan fee will also be refunded.

No refunds are issued after 14 days or once a claim has been submitted.
11. Limitation of Liability
LifeArt Vision shall not be liable for:

Indirect, incidental, or consequential damages

Loss of use, time, or productivity
-
Personal injury or property damage resulting from the use of any eyewear
-
Claims related to shipping, customs delays, or acts of God
The Company’s total liability under this Plan shall not exceed the original retail value of the covered product.
12. Governing Law and Dispute Resolution
This Plan shall be governed by the laws of the State of [Insert State], without regard to conflict of law principles. Any dispute arising under this Plan shall be subject to mandatory arbitration under the rules of the American Arbitration Association.
13. Modifications
LifeArt Vision reserves the right to update, modify, or terminate this Plan at any time. Changes will not affect Plans already purchased and in effect.
